• Utility bill (electricity / water) not older than 2 months

• Layout plan of the unit / premises

• List of machinery and equipment used

• List of food products handled or manufactured

• NOC from the local authority / municipality

• Water testing report from a recognized laboratory

• Food Safety Management Plan (FSMS)

3. Additional (if applicable)

• License from local body (Municipal Trade License)

• DGFT certificate (for exporters/importers)

• Nomination of authorized person (Form IX)

• Employee medical fitness certificate
